Electrocardiographic Manifestations and Clinical Outcomes of Severe Hyperkalemia: Can the Electrocardiogram Risk Stratify for Short Term Adverse Events?
نویسندگان
چکیده
Hyperkalemia produces cardiotoxicity through early depolarization of the cell membrane, slowing of ventricular conduction and decreased duration of the action potential. As a patient’s potassium rises, standard teaching describes the sequential appearance of classic electrocardiographic (ECG) abnormalities. Initially peaked T waves are observed. Next, the PR interval prolongs, followed by QRS prolongation. Then, there is loss of the P wave and escape rhythms are seen. Finally, the ECG develops a “sine wave” configuration, leading to ventricular fibrillation, pulseless electrical activity or asystole.

Severe Hyperkalemia: Can the Electrocardiogram Risk Stratify for Short-term Adverse Events?
INTRODUCTION The electrocardiogram (ECG) is often used to identify which hyperkalemic patients are at risk for adverse events. However, there is a paucity of evidence to support this practice. This study analyzes the association between specific hyperkalemic ECG abnormalities and the development of short-term adverse events in patients with severe hyperkalemia. METHODS We collected records of...
متن کاملElectrocardiographic left ventricular hypertrophy is not associated with increased in-hospital adverse events in patients experiencing first non-ST segment elevation myocardial infarction: A single center study
Background: There is conflicting data about prognostic implication of electrocardiographic (ECG) left ventricular hypertrophy (LVH) in patients with first non- ST-segment elevation myocardial infarction (NSTEMI). We aimed to examine the association of left ventricular hypertrophy (LVH) on admission electrocardiogram with adverse outcomes in patients with NSTEMI. Methods: In the present study, ...
متن کاملRetrospective review of the frequency of ECG changes in hyperkalemia.
BACKGROUND AND OBJECTIVES Experimentally elevated potassium causes a clear pattern of electrocardiographic changes, but, clinically, the reliability of this pattern is unclear. Case reports suggest patients with renal insufficiency may have no electrocardiographic changes despite markedly elevated serum potassium. In a prospective series, 46% of patients with hyperkalemia were noted to have ele...
متن کاملUnsupervised Similarity-Based Risk Stratification for Cardiovascular Events Using Long-Term Time-Series Data Citation
In medicine, one often bases decisions upon a comparative analysis of patient data. In this paper, we build upon this observation and describe similarity-based algorithms to risk stratify patients for major adverse cardiac events. We evolve the traditional approach of comparing patient data in two ways. First, we propose similarity-based algorithms that compare patients in terms of their long-t...
